경추 전방 디스크 제거수술 ACDF (anterior cervical discectomy & fusion)
1. 적응증
경추간판탈출증, 경추추간공협착증, 경추관 협착증
2. disc 진행단계
■ 팽윤 (degenerative disc)
디스크 수핵이 뒤쪽으로 약간 밀려있는 상태 / 수핵이 신경압박X / 섬유륜 파열X
■ 돌출 (bulging disc)
디스크 수핵이 심하게 밀려있는 상태 / 돌출된 수핵이 신경압박O / 섬유륜 파열X / PRN) 증상에 따라 수술
■ 탈출 (herniatied disc)
디스크 수핵이 터져나온 상태 / 수핵이 외부로 돌출된 상태 / 섬유륜 파열O / PRN) 증상에 따라 수술
■ 얇아진 디스크 (thinning disc) → disc degeneration with osteophyte formation = 뼈돌기 bone spurs
디스크 높이도 낮아지고, 뼈의 자극으로 인한 뼈돌기(모양이 울퉁불퉁해짐) / 수술필요
3. 경추 전방 디스크 제거수술 ACDF (anterior cervical discectomy & fusion)
= Anterior 전방 + Cervical 경추 + Discectomy 추간판제거 + Fusion 유합
= 경추(목, cervical)의 앞쪽(anterior)으로 접근하여 두 주체(vertebra) 사이의 추간판(vertebral disc)을 제거함(discectomy)과 동시에 유합(fusion)하는 수술
= 간략하게 말해, 목디스크 수술
4. procedure
밖 : C-arm 보면서 level check : K-wire + 마킹펜
Drap : Soap → ST → Solu → HB + 코카
1. incision ) #15 → bipolar(blunt) + ADS teeth
2. 양쪽 retraction ) allis * 2
3. dissection ) platysma(광견근)의 fascia NBT로 박리
4. 시야확보 retraction ) fascia peanut + kelly로 닦음 → weithrenur로 retraction(시야확보)
5. vertebral body dissection ) 손가락으로 dissection, 벌리면서 vertebral body 만져보기 → double, metezenbaum → Rt.angle ret, RBT, #15
6. disc level check ) kelly or double 위치잡고 c-arm ON → 정확한 위치 마킹펜으로 표시
8. 좌우 casper ret ) blade걸고 self ret 걸기
9. 상하 casper holding arm & 고정pin박기 ) 뭉치 → ioban 남은거 → 고정봉 → drill 20D(pin 박을 자리) → pin+driver 합체해서 주기 *2(양쪽) → self ret
10. conta 방지 ) L-utility로 뭉치,고정봉 부분 붙여서 C-arm lateral시 conta방지
11. 현미경 IN
1) incision ) long mess H + #15로 disc 절개
2) vertebral endplaste(추골종판) drill ) 30cc D → 30cc irrigation / 깊어지면 TSA burr
3) dissetion & disc 제거 ) hook, kerrison punch, pituitary rongeur, double
12. 현미경 OUT / trial size측정 ) trial 순서대로 힘받는지 확인해서 맞는 trial size 찾기
13. irrigation & cage insertion ) size 정해지면 Spoid irrigation → CAGE(인공뼈) 넣고 mallet으로 두드림 → 남은DBM
14. pin remove ) pin driver로 pin빼면 bone wax로 채우기
15. plate & screw ) vertebral body에 plate대고 screw박아서 고정 (screw 박을 때 drill 20D사용)
16 bleeding control ) bipolar, 지혈제 사용
17. saline irrigation & drain insertion
18. closure Sx ) St.mastoid ret 걸고 fascia v, subcu v, steri-strip 1/3등분
19. Dx ) dry 4*4, oint

■ 디스크 돌출(bulging disc), 디스크 탈출(herniated disc), 뼈돌기(bone spurs)로 인해 척수 압박(cord compression), 후종인대 압박 PLL(posterior longitudinal ligament)
└ disc 악화 증상 : 목통증(neck pain), 뻣뻣함(stiffness), 무감각(numbness), 팔, 손의 허약감(weakness in your arms or hands)
└ 이유 : 척추관이 좁아져서 신경압박으로 인함
■ disc 제거이유
: 손상된 disc를 제거하게 되면, 척수와 신경뿌리에 가해지는 압력완화되기 때문이다.
■ disc 후 뼈 다지는 이유
: 제거된 디스크 위와 아래의 척추체를 다듬어 뼈이식(bone graft+CAGE)하기 위함
■ 후종인대 PLL(posterior longitudinal ligament) ?
: 척추뼈를 연결하는 인대로 척추사이 움직임을 정상적으로 유지하는 역할을 한다.
→ 연화증(굳어지는 것)이 생기면 척추관을 지나가는 척추신경 압박하게 되어 신경장애증상을 유발한다.
